Application forms may also be downloaded from the website of this directorate. Print out of the application form (in Legal size bond papers) may be filled up and submitted to the Directorate of Health Services, Lamphelpat within the timeline along with cash payment of Rs. 1000/- (Rs. 700/- only for SC/ ST and PWD).
