Sealed quotation affixing non-refundable Court fee stamp of Rs.8.25 (Rupees eight and twenty five paisa) only are invited from reputed Govt. Registered Supplier for the supply of N95 Face Mask (specifications are as follows) which are urgently required at Fakhruddin Ali Ahmed Medical College & Hospital, Barpeta related to COVID-19 pandemic.

1. Court Fee Stamp of Rs. 8.25
2. GST Registration Certificate.
3. GST Return (up-to-date).
4. PAN Card
5. IT Return of the last assessment year.
6. Trade License (up-to-date)
7. Sample(s) must be submitted with the SQN.

The quotations are to be submitted to the Superintendent, F.A.A. Medical College Hospital, Barpeta within 07 (seven) days from the date of the issue of this Quotation during office hours. Quotation will be opened on 14th October, 2020 at 11:30 A.M. Quoted rate(s) should be inclusive of all taxes.
