Unique Disability ID Apply: A UDID (Unique Disability ID) card is a unique ID for Persons with Disabilities (PwDs). It's a single document for the identification and verification of PwDs for access to various benefits.
Visit the UDID Web portal and click on the 'register link' to register. In the head 'types of details', give personal details including address, disability details and employment details. After entering details, upload your colour passport size photo and other requisite documents, and submit all data. The CMO (Chief Minister's Office) / medical authority will verify your data and assign specialists for the assessment of disability. A specialist doctor gives an opinion on disability. The medical board reviews the case and ascertains the disability percentage. The CMO prepares disability certificates and generates UDID and certificates.
The Department of Empowerment of Persons with Disabilities (PwDs), Government of India has issued gazette notification SO 1736(E) on May 5, 2021, making it mandatory for all States/UTs to grant certificates of disability through online mode only using UDID portal with effect from June 1, 2021.
