Information About How To Get Next of Kin Certificate

Want the certificate of next of kin (NOK) of any of your deceased relatives? You need to apply for it either through a PFC (public facilitation centre) or a CSC (common service centre) with an application fee of Rs 60. CFCs are attached to Deputy Commissioner’s office, sub-divisional officer’s office and revenue circle office. CSCs, on the other hand, are found almost everywhere. Officials manning the PFC or the CSC you approach will upload your application in the website – www.assam.gov.in. You can also get it done yourself by visiting www.assam.gov.in. You need to register yourself as a citizen in the website. The options available there will lead you to your destination – NOK.

It seems very simple. However, you will have to give your details – date of birth, father’s name, mother’s name, name of the spouse and the permanent address in the application. The permanent address includes the State, district, police station and the mouza you live in, besides the PIN code.

You will also have to give detailed information of the deceased person that include – name of the deceased, his/her father’s/guardian’s name, date of death, place of death, reason of death, your (applicant’s) relation with the deceased and the address of the deceased.

These apart, you will have to give the family details of the deceased – name of kin, his/her relation with the deceased and his/her age as on the date of application.

All the above information should be backed by supporting documents like the death certificate, an affidavit of the applicant, a document showing the relation of the applicant with the deceased and the identity proof of the applicant.

The certificate will be issued within a month from the date of application.
